Healthcare Services General Fees
THERE IS NO FEE FOR SERVICE FOR PATIENT PHYSICAL ASSESSMENT AND EXAMINATION
Outside charges can occur if the following services are provided:
- Laboratory (i.e. blood tests, cultures) – a bill will be sent
from the outside laboratory to what ever billing address is provided
- Pharmacy – your bursar account will be charged (limited
prescriptive medications at reduced cost are available)
- Routine physical examinations [excluding entrance examinations]
$75.00 charge to your bursar account; you may bill your Insurance.
Loan items will be charged to your bursar account if they are not returned within two weeks. These items include:
- Pair of crutches - $75.00
- One crutch - $40.00
- Heating pad - $35.00
**ALL FEES AND INSURANCE CLAIMS ARE THE RESPONSIBILITY OF PATIENT**
